Questions 5 and 7 - scattering theory is very well covered in Sakurai's 'Modern Quantum Mechanics', chapter 7. In general, this book is worth a look to make the link between basic QM and scattering theory, form factors etc (which turn out to be based on straightforward QM with a few approximations). Gives derivations of amplitudes for Born and partial-wave approximations.

For a good treatment of the square potential barrier (relevant for q7), see section 5.3 of James Binney's QM textbook, used in the Oxford course.
